Use the attached files to base the appendix off of.
Required Appendices:
– Instruments/Measures
– Include the full-scale/version that shows all the questions/items.
– Type up the questions and items on the document.
– Include the written instructions or prompt for each measure that participants would see if they were to complete the study.
– Appendices need to be in the same order that participants are viewing and as described in the procedure written section
– Example measures to include in Appendix
– Participation criteria questions/items (if applicable)
– Independent variable(s) measure
– Dependent variable(s) measure
– Manipulation check questions/items (if applicable)
– Demographic items (at a minimum must include age, race, and gender)
– Any other demographics that pertain to your study
– Age – open-ended in years typically (recommended to use a
quantitative scale of measurement)
– Race and gender – multiple-choice items (recommended to use a
quantitative scale of measurement)
– Debrief Statement (optional)
– This is usually a paragraph or two sentences.
– Briefly provide a broad description of the real purpose of the study since you
don’t want to give away the real purpose in the beginning in your consent form
description (sampling error)
– Request that participants do not share information with others
– Allow participants to view results (optional)
– Thank them for participating
– Include your contact information if they have any follow-up questions
